§ 15491. Group Self Insurer and Group Member Application Filing Fees.

- (a) Each group self insurer and/or group member making application for a Certificate of Consent to Self Insure or requesting an Interim Certificate of Consent to Self Insure shall pay a non-refundable filing fee at the time of filing the application or requesting the Interim Certificate on the following basis:(1) A filing fee to accompany a new group self insurer application shall be $1,000 for the group application and an additional $500 for each group member application submitted with the group self insurer's application;(2) For each additional group member's application, the filing fee shall be $500 if no Interim Certificate has previously been issued to the applicant or $400 if an Interim Certificate has been issued and is in effect at the time the application is submitted;(3) For each Request for an Interim Certificate submitted pursuant to Section 15482.1 of these regulations, the filing fee shall be $100.(b) Any subsequent re-filing of an application by an existing group self insurer or group member following a merger, acquisition, or reincorporation shall be considered a new group self insurer or group member application and each such new filing shall be subject to a filing fee pursuant to subsections (a)(1) through (a)(3) of this section.HISTORY1.
