§ 9796. Certification of Consulting Physician, How Initiated.

- When an injured employee requests an employer to secure certification of a consulting physician under Labor Code Section 4602, the employer shall direct a letter in triplicate to the Division of Industrial Accidents, attention Medical Director, 525 Golden Gate Avenue, Room 201, San Francisco, California 94102, containing the following information:(a) The name and address of the injured employee;(b) The name and address of the consulting physician chosen;(c) The field of practice of the consulting physician.HISTORY1. Amendment filed 11-7-78; effective thirtieth day thereafter (Register 78, No. 45).2. Change without regulatory effect filed 7-11-86; effective upon filing (Register 86, No. 28).
