§ 35002. Initialing the Relinquishment or Consent Form.

  • (a) The relinquishment or consent form shall not be corrected or altered unless the correction or alteration is initialed by:
    (1) The parent who signed the form.
    (2) The reader or translator, if one was used.
    (3) The authorized representative of the agency.
    (4) The witnesses to the relinquishment.
